Art in the Park Registration

The registration form can be completed and submitted in variety of ways:

 Print PDF, Fill-In and Submit by one of the following:

  1. In person to 407 Railroad St, Elko.
  2. Via postal mail to PO Box 1381, Elko, NV 89803
  3. Via photo or scan to elkoaip@elkocountyarts.org or text to 775-544-5198

Please note, only certified service animals are allowed at Art in the Park!

Permits and Tax Information

*All vendors, please mark each permit / ID you have and fill-in the number. **A temporary business license will be provided if you do not have one. ***If you do not have a Tax Identification number, leave the space below blank. DO NOT enter your Social Security number below, only enter it on the NV State form only.

Liability Insurance

Food and Activity Vendors please add as an additional insured: Elko County Art Club, Art in the Park (event location: 1467 Idaho Street, Elko, NV 89803); a copy with the added insurer emailed to ElkoAIP@ElkoCountyArts.org. If you don't have liability insurance contact us directly.

Booth Space

All booths are 10' deep.
